​As cancer treatments improve, more and more cancer patients are making it to the end of treatment and will no longer require hospital care. Shari Moura is a clinical nurse specialist working to help survivors make a smooth transition back to their family doctors through a pilot project called the After Cancer Treatment Transition Clinic (ACTT). ​I sat down with Shari to discuss the importance of partnering with family physicians to ensure good mangement of long term side effects, chronic disease, and monitoring for cancer recurrence.
